INSTRUCTIONS:

1. Click the button below 
2. Click "Add Team Member" to start
3. If needed, delete any placeholder team members

Your info will be saved on your computer as you go. Next time you visit and click the button, everything will still be right where you left it.

TO SAVE THE FILE TO YOUR COMPUTER:

1. Right-click the button below
2. Choose "Save Link As..." or “Download Linked File As…”
3. Choose where to save (I suggest "Desktop")
4. Click "Save"
